You’ve spent a lifetime building your home. It’s where you’re comfortable. Relaxed. At your best. There, surrounded by memories is where you want to be, and we want you to continue living there as long as possible. Our Immanuel Pathways program brings the care to you; in your home and in the PACE® Center, so all your needs are met.

This non-profit Program of All-inclusive Care for the Elderly (PACE®) offers a complete health plan for eligible individuals. Our comprehensive team of health-care professionals manages not only your well-being but also your connection to the community and the life you’ve made in your home. It’s a more personal and complete way of getting health care.

Locations

Immanuel Pathways currently serves seniors in three areas:

Eastern Nebraska in select zip codes of Douglas, Sarpy, Cass, Dodge, Saunders and Washington counties.

Southwest Iowa: in Mills, Harrison and Pottawattamie counties.

Central Iowa: in Boone, Dallas, Jasper, Madison, Marion, Marshall, Polk, Story and Warren counties.

Care & Services

We want you to live in your home for as long as possible. To accomplish that goal, we provide a wide range of care to help you remain active and healthy. We encourage and cover preventative care, as well as all the services covered by Medicare and Medicaid as authorized by our Interdisciplinary Team.

Immanuel Pathways is a provider-sponsored health plan, which means you’ll work with your PACE® primary care providers and specialists to make decisions about your health care. It is your decision, and we’re here to make it happen.

  • Transportation to the PACE® Center and medical appointments
  • Home Care
  • Primary Care
  • Prescription Drugs
  • Emergency Services
  • Physical Therapy
  • Occupational Therapy
  • Dentistry
  • Social Services
  • Nutritional Counseling
  • Hospital Care
  • Nursing Home Care

Eligibility

  • You may enroll in Immanuel Pathways if you meet the following:
  • You are 55 years or older.
  • You live in an Immanuel Pathways service area.
  • You are assessed to meet nursing facility level of care.
  • You are able to live safely in the community with the support of Immanuel Pathways services.

Costs

Immanuel Pathways charges no deductibles or copayments for any drug, service or care approved by the PACE® Interdisciplinary Team.

If you are eligible for Medicaid and Medicare, you pay no premium for the Immanuel Pathways services.

If you are eligible for Medicaid only, you pay no premium for Immanuel Pathways services.

If you are eligible for Medicare only, you will be charged a monthly premium for the program, which includes Medicare Part D.

If you are not eligible for Medicare or Medicaid, Immanuel Pathways staff will discuss a private pay rate with you.

PACE® participants may be fully and personally liable for the costs of unauthorized or out-of-PACE® program agreement services. Emergency services are covered.
